Welcome to another STC where friend of the podcast, Josh Inglis, challenges us with a case from Australia.  Play along with us and try to get the diagnosis as early as possible.

As an aside, our update regarding COVID19 in NZ is already a bit out date.  Time move quickly during a pandemic!  NZ has not only managed to “flatten the curve”; we have completely squashed it.  For each of the last 4 days we have had less than 15 new confirmed and probable cases.  Now we have to figure out how to stay COVID-free until the vaccine…

Dear Art and Nick, thank you for running though this very interesting case. I am an advanced trainee in Infectious diseases and General medicine, based in Queensland Australia. I have had the privilege of working at a cardiothoracic centre during my training time. Whipple’s disease is not common and requires a high index of suspicion to diagnose. Just an additional comment that T.whipplei is known to be a culprit for culture negative endocarditis and is part of my algorithm for approaching possible endocarditis, where I clinically suspect it and other cultures are negative.
